New Patient Registration

In order to register with the practice please complete a registration form which you can request by calling the Health Centre or by downloading from the Registering Online tab. The completed forms should be posted to Peterhead Health Centre, Links Terrace, Peterhead, AB42 2XA or they can be emailed to gram.peterheadadministrator@nhs.scot 

Once completed, your details will be added onto the computer and you will become a member of our practice. Patients on potentially abusable medications may need to see their registered GP prior to a decision being made on whether these medications will be continued or not.

 

PLEASE NOTE this practice does not prescribe Methadone (or suboxone), but would refer any patients on this medication to the specialist substance misuse services. Any patients affected by this who wish to register at this practice must ensure they have alternative arrangements in place until they are seen by the specialist services.
